The PFM Foundation welcomes applications at any time. The process to submit applications is easy:
- Download the PDF application.
- Review the detailed instructions included within the application.
- Provide the requested information. Please be sure to provide as much detail as possible to help us process your request.
- Remember to include any documentation that supports your request. Supporting documentation may include copies of bills/invoices, notice evidencing loss of income, and/or other evidence of the qualifying event or need.

The application process typically takes 2-3 weeks. You will be contacted regarding the Foundation’s decision within 30 days of receipt of your completed application.
